The Social Care Institute of Excellence (SCIE) and the National Institute of Care and Excellence (NICE), have co-produced this information based on NICE’s guidelines and quality standards on Intermediate care, including reablement.
Intermediate care involves working closely with people to agree what intensive support they need to improve their independence. Support should focus on the person’s strengths and help them to develop resilience and confidence so that they feel more able to manage daily living. This quick guide provides an overview of planning and delivering person-centred intermediate care. It covers:
- Core principles
- Person-centred planning
- Personalised goals
